IVR Process Can Challenge Sex LifeA new study discovers that infertility treatment can negatively impact a women?s sex life. Despite the importance of sex in conceiving a child, little attention has been given to the sexual dynamics of couples as they work to overcome infertility challenges.

?Sex is for pleasure and for reproduction, but attention to pleasure often goes by the wayside for people struggling to conceive,? said Nicole Smith, a doctoral student with Indiana University?s Center for Sexual Health Promotion.

Smith is conducting the study in collaboration with Jody Lyne? Madeira, Ph.D.

?With assisted reproductive technologies (ART), couples often report that they feel like a science experiment, as hormones are administered and sex has to be planned and timed. It can become stressful and is often very unromantic and regimented; relationships are known to suffer during the process.?

Researchers say the study is one of the first in the United States to examine women?s sexual experiences while undergoing assisted reproductive technologies. Investigators used a Sexual Functioning Questionnaire to assess the impact of IVF treatment on couples? sexual experiences.

Compared to a sample of healthy women, women undergoing IVF reported significantly less sexual desire, interest in sexual activity and satisfaction with their sexual relationship. They had more difficulty with orgasm and were more likely to report sexual problems such as vaginal pain and dryness.

Experts have recognized that emotional and relationship challenges intensify as a couple?s use of ART proceeded. Attention to challenges in having sex has somehow escaped the purview of key parties.

When couples meet with their physicians, their sex life might not top the list of issues they want to discuss, either because of unease talking about the subject or simply because they have so many other important issues to discuss.

Still, Smith and Madeira say, the doctor-patient relationship is key, and couples can be told up front about the potential sexual side effects and resources that can help.

If they have issues with dryness, for example, they could be counseled on remedies such as purchasing lubricant or other sexual enhancement products. In addition to referring couples to mental health counselors, reproductive endocrinologists could also refer them to sex therapists.

?There?s just a dearth of knowledge on how infertility affects sexual behavior,? Madeira said. ?The focus is more likely to be on the social and support dimensions of the relationship, but sex is a big part of that. Just letting patients know they aren?t alone in this would be helpful.?

If more information about sexual challenges becomes available, couples might find it on their own.

?Women interested in ART are generally well-educated and tend to spend time researching these issues,? Madeira said. ?They would be very responsive to this information, and proactive.?

The study involved 270 women who completed an online questionnaire; interviews with 127 men and women using IVF to try to conceive; and interviews with 70 professionals, including physicians, nurses, mental health experts and other providers who work directly with patients.

IVF is a procedure in which mature eggs are retrieved from a woman?s ovaries and fertilized by sperm in a lab, forming embryos. The embryo(s) are then implanted in the woman?s uterus. It is considered an effective procedure but one that is used after couples try several other less invasive procedures.

By the time couples begin IVF, they might have been trying to conceive for many years. Nine percent of the women in their study had been through five IVF cycles, which could take at least a year.

Here are some of their other findings:

  • Women who reported being sexually active with a partner in the past month also were more likely to engage in masturbation and report fewer sexual problems;
  • The women reported similar problems with sexual function regardless of the type or source of infertility involved: male factor, female factor, or both male and female factor;
  • Hormonal treatments used in assisted reproductive technologies likely affect women?s sexual experiences and pain, but these effects are not as well understood and receive less priority than other conditions, such as heart disease and cancer.

Wireless Computers: Formerly A Luxury, Now A ... - Retirement Homes

It?s sometimes easy to forget how much technology has changed in the last 10 years. Personally, I grew up with a generation that had Internet access from the moment it was available to everyone, nearly 2/3 of my life. At the beginning, getting online was much different. Our family computer was a $3000, large and bulky, tan colored machine that had its own room in our house. It used a loud modem to connect to the only phone line we had, which meant no one could make a call while the Internet was in use.

Today, getting online is easier and more convenient than ever before. With the introduction of WIFI, which is the name commonly given to the concept of connecting to the Internet with a device wirelessly using radio waves, everything changed. Our computers (laptops) got smaller and more powerful, which promoted using these tools in a more mobile way. We were no longer forced to sit in our computer rooms anymore. We had the freedom to sit on the couch and check our email, or watch a cooking tutorial while preparing dinner in the kitchen. WIFI made it possible for the Internet to be available to everyone virtually anywhere. What a game changer!

Connecting to the Internet using WIFI is also the technology that makes new gadgets such as tablets, smart phones, and new laptops possible, as these devices do not have another means to get online. So, when you walk around your retirement home sharing photos of your grandchildren on Facebook with your neighbors, imagine trying to do the same with a long wire attached to the wall. It would actually seem ridiculous. Future technologies will undoubtedly all use WIFI to access the Internet. Wires will soon be a thing of the past.

Because WIFI is so common, it?s important that everyone understands a few basic concepts about it, to use it confidently and safely. First, if you have your own WIFI connection at home, be sure to setup a WIFI password. This will ensure that only the people who know your password can get access to you WIFI connection. Second, when using public WIFI, like at Starbucks or your modernized retirement home, avoid making online purchases or doing online banking. With no WIFI password, anyone could view all the activity on the public WIFI connection, which includes your credit card number and bank information. Reserve this type of online activity for at home, when you?re using a private, password protected WIFI connection.

Finally, it?s important to know how to make your WIFI connection signal as strong as possible, so your Internet loads quickly. Be sure to have the WIFI router (electronic box with an antenna that transmits wireless radio waves) at a high elevation, wherever it?s setup. Also, keep it away from other electronic devices. This will help create a wide open area for the WIFI Internet connection to be projected and will decrease the amount of electronic interference other technologies might create.

Have fun out there in our WIFI enabled world. Just imagine when entire cities offer free WIFI to everyone?wow! WIFI truly is a technology for seniors.

(AP) ? When Tom Sermanni shook hands with Pia Sundhage after her last game as coach of the U.S. women's soccer team, he should have asked her for a few tips.

Sermanni was hired Tuesday to replace Sundhage, who led the Americans to back-to-back Olympic gold medals and their first World Cup final in 12 years. Sermanni has spent the last eight years as Australia's coach, taking the Matildas to the quarterfinals of the last two Women's World Cups.

"He has the knowledge, experience and vision to take on the challenge of keeping our team at the top of the world," U.S. Soccer Federation president Sunil Gulati said in a statement. "He has a tremendous passion for the game, knows the American players, understands our system and knows the process of preparing a team for a World Cup tournament."

Sermanni, a 58-year-old originally from Glasgow, Scotland, has spent much of the last 20 years in Australia, where he is credited with transforming the Matildas into one of the world's top programs. In addition to their quarterfinal appearances at the 2007 and 2011 World Cups, the young Australians won the 2010 Asian Women's Cup. Their runner-up finish at the same tournament in 2006 earned Sermanni Asian Football Confederation coach of the year honors.

The Australians are ninth in the world, matching their highest ranking.

Sermanni also coached Australia from 1994-97, leading the Matildas to their first appearance in a World Cup, the 1995 tournament in Sweden. He was among 10 candidates for FIFA's 2011 Women's Coach of the Year.

"Tom's legacy for Women's Football in Australia will be felt for many years to come as he has helped transform our women's national team to a very professional and competitive group of players," Ben Buckley, CEO of Football Federation Australia, said in a statement. "Tom will always be considered a close friend of Australian football no matter where he is in the world."

Sermanni was selected after a five-person search committee, which included Mia Hamm, considered more than 30 candidates. The current U.S. players said they didn't care whether the new coach was male or female, American or foreign-born, so long as the right coach was selected.

The coaches of both U.S. national teams are foreign-born, with German-born Jurgen Klinsmann, a longtime California resident, coaching the men.

"Someone who's good enough, that's all I care about," Abby Wambach said earlier this month. "Be the person who brings the World Cup back."

Sundhage was 91-6-10 in her five years with the Americans, including a 23-1-1 record this year, and the U.S. was ranked No. 1 in the world for most of her tenure. She resigned Sept. 1 to return to her native Sweden, where she is now the women's national team coach.

"We need a strong person to come in and add to the history that this team, specifically, has created in the last two years," Wambach said. "... This team is scary good, and we need to have someone who can put all the Xs and Os together."

After Sermanni's hiring was announced, Megan Rapinoe, Alex Morgan and Wambach welcomed him via Twitter.

"really pumped about (at)TomSermanni being the new uswnt head coach!!! Welcome. (hashtag)movingforward," Wambach said.

Sermanni has experience coaching young players, which will be key as the U.S. begins integrating its next generation. Though veterans like Wambach, captain Christie Rampone and Shannon Boxx have all talked of playing through the 2015 World Cup in Canada, the next major tournament, there is plenty of depth and talent at the lower levels, too. The U-20 team just won the World Cup, and the U-23 team won both tournaments it played this year.

Sundhage had also begun changing the Americans' style of play. As the game evolves and improves around the world, relying on the advantages it has in size, speed and power is no longer enough for the U.S. Sundhage introduced a Barcelona-style attack that relied on creativity and ball possession, and Sermanni will have to decide whether to continue that or not.

"U.S. Soccer has always been at the forefront of supporting the women's game, and it's exciting to coach the team in this next chapter of its history," Sermanni said. "After coaching against many of these players for years, I am looking forward to working with an accomplished group of veterans while integrating the numerous talented young players who are itching for a chance to prove themselves.

"I'm honored to have this opportunity to work with tremendous players and in a program that has had such a tradition of success," he added.

Sermanni does not take over officially until Jan. 1, and will coach Australia in the East Asian Cup Qualification tournament from Nov. 20-24 in Shenzen, China. Interim coach Jill Ellis will lead the Americans in exhibitions against Ireland on Nov. 28 in Portland, Ore., and on Dec. 1 in Glendale, Ariz., as well as three games that have yet to be announced.

A midfielder, Sermanni played professionally from 1971-89 at clubs in Scotland, England, Australia and New Zealand, including Blackpool and Torquay, and scored more than 50 goals. He got into coaching soon after, and has had stints as both a men's and women's coach in Japan, the U.S. and Malaysia, in addition to Australia.

Study reveals impact of public DNS services; Researchers develop tool to help

ScienceDaily (Oct. 25, 2012) ? A new study by Northwestern University researchers has revealed that public DNS services could actually slow down users' web-surfing experience. As a result, researchers have developed a solution to help avoid such an impact: a tool called namehelp that could speed web performance by 40 percent.

Through a large-scale study involving more than 10,000 hosts across nearly 100 countries, Fabi?n Bustamante, associate professor of electrical engineering and computer science at Northwestern's McCormick School of Engineering and Applied Science, and his team found that one cause of slow web performance is a growing trend toward public Domain Name Systems (DNS), a form of database that translates Internet domain and host names into Internet Protocol (IP) addresses.

DNS services play a vital role in the Internet: every time a user visits a website, chats with friends, or sends email, his computer performs DNS look-ups before setting up a connection. Complex web pages often require multiple DNS look-ups before they start loading, so users' computers may perform hundreds of DNS look-ups a day. Most users are unaware of DNS, since Internet Service Providers (ISP) typically offer the service transparently.

Over the last few years, companies such as Google, OpenDNS, and Norton DNS have begun offering "public" DNS services. While "private" DNS services, such as those offered by ISPs, may be misconfigured, respond slowly to queries, and go down more often, public DNS services offer increased security and privacy, and quicker resolution time. The arrangement is also beneficial for public DNS providers, who gain access to information about users' web habits.

Bustamante and his team found that while using public DNS services may provide many benefits, users' web performance can suffer due to the hidden interaction of DNS with Content Delivery Networks (CDNs), another useful and equally transparent service in the web.

CDNs help performance by offering exact replicas of website content in hundreds or thousands of computer servers around the world; when a user types in a web address, he is directed to the copy geographically closest to him. Most popular websites -- more than 70 percent of the top 1,000 most popular sites, according to the Northwestern study -- rely on CDNs to deliver their content quickly to users around the world.

But researchers found that using public DNS services can result in bad redirections, sending users to content from CDN replicas that are three times farther away than necessary.

Public DNS and CDN services are working to address the problem, but current users are left with two mediocre options -- bad web performance through public DNS services or bad security and privacy support through private DNS services.

Now Bustamante and his group have developed a tool called namehelp that may let users have their cake and eat it, too -- by using public DNS services without compromising on web performance.

namehelp runs personalized benchmarks in the background, from within users' computers, to determine their optimal DNS configuration and improve their web experience by helping sites load faster. If it finds that a user is receiving less than optimal web performance, namehelp automatically fixes it by cleverly interacting with DNS services and CDNs to ensure the user gets his content from the nearest possible copy.

You can download namehelp today from: http://aqualab.cs.northwestern.edu/projects/namehelp.

The paper describing the research is titled "Content Delivery and the Natural Evolution of DNS: Remote DNS Trends, Performance Issues and Alternative Solutions." The team's findings will be presented at the Internet Measurement Conference (IMC 2012) in Boston this November. In addition to Bustamante, authors on the paper are lead author John S. Otto, Mario A. Sanchez, and John P. Rula, all of Northwestern.

Gene mutation identifies colorectal cancer patients who live longer with aspirin therapy

ScienceDaily (Oct. 24, 2012) ? Aspirin therapy can extend the life of colorectal cancer patients whose tumors carry a mutation in a key gene, but has no effect on patients who lack the mutation, Dana-Farber Cancer Institute scientists report in the Oct. 25 issue of the New England Journal of Medicine.

In a study involving more than 900 patients with colorectal cancer, the researchers found that, for patients whose tumors harbored a mutation in the gene PIK3CA, aspirin use produced a sharp jump in survival: five years after diagnosis, 97 percent of those taking aspirin were still alive, compared to 74 percent of those not using aspirin. By contrast, aspirin had no impact on five-year survival rates among patients without a PIK3CA mutation.

"Our results suggest that aspirin can be particularly effective in prolonging survival among patients whose colorectal cancer tests positive for a mutation in PIK3CA," said the study's senior author, Shuji Ogino, MD, PhD, of Dana-Farber, Brigham and Women's Hospital, and the Harvard School of Public Health. "For the first time, we have a genetic marker that can help doctors determine which colorectal cancers are likely to respond to a particular therapy." He cautions that the results need to be replicated by other researchers before they can be considered definitive.

While aspirin is often prescribed for colorectal cancer patients, doctors haven't been able to predict which patients will actually benefit from the treatment. The new finding suggests that the survival benefit is limited to the 20 percent whose tumors have the PIK3CA mutation.

For the remaining patients, aspirin may still be used, but it is likely to be much less effective and can sometimes lead to gastrointestinal ulcers and stomach bleeding.

The study was prompted by previous research that suggested that aspirin blocks an enzyme called PTGS2 (cyclooxygenase-2), causing a slowdown in the signaling activity of another enzyme, PI3K. That led researchers to hypothesize that aspirin could be especially effective against colorectal cancers in which the PIK3CA gene -- which provides a subunit of PI3K -- is mutated.

To conduct the study, investigators obtained data on 964 patients with rectal or colon cancer from the Nurses' Health Study and the Health Professionals Follow-up Study -- long-term tracking studies of the health of tens of thousands of people. The data included information on the patients' use of aspirin after diagnosis and the presence or absence of PIK3CA mutations in their tumor tissue.

The study, which combines the study of disease-related genes and research into large populations of individuals, represents a new, hybrid field which Ogino has termed molecular pathology epidemiology. "The field may help us bring together information from two frontiers of cancer research -- at both the molecular and population levels -- in ways that are beneficial to patients," he said.

The study was supported in part by grants from the National Institutes of Health (P01 CA87969, P01 CA55075, P50 CA127003, R01 CA149222, R01 CA137178, and R01 CA151993); the Bennett Family Fund for Targeted Therapies Research; the Entertainment Industry Foundation through the National Colorectal Cancer Research Alliance; the Frank Knox Memorial Fellowship at Harvard University; and a Damon Runyon Clinical Investigator Award.

Adidas wins latest legal round in lightweight shoe battle vs. Nike ...

Adidas appears to have won the latest round in the legal tangle over Nike's contention that Adidas infringed on Nike's development of a super-lightweight running shoe.

The Nuremberg District Court today said "the granted injunction cannot be sustained," according to a statement issued by Germany-based Adidas. "It is to be expected that on November 7, 2012, the Court will set aside its initial decision and decide in favour of adidas."

?Nike, the Oregon-based company, sought and obtained an injunction Aug. 28, alleging the adizero Primeknit was a copy of the Nike Flyknit.

Nike, the world's largest sporting goods company, contended that its Flyknit running shoes were the first to offer a technology in which woven threads formed the shoe's upper. Nike executives have said the Flyknit technology has far-reaching potential, from altering shoe production methods to curtailing shoe construction waste.

The Flyknit running shoe, which has a line that was co-developed by Nike chief executive Mark Parker, was introduced at a Summer Olympics-themed event in New York in February.

But on July 25, about a week before the Summer Olympics' opening, Adidas, the world's second-largest sporting goods company, introduced the Primeknit shoe, a remarkably similar product to Nike's Flyknit.

Adidas, in its statement today, said the original Aug. 28 injunction was set aside just days after it was granted in advance of today's hearing. ?

"Adidas vigorously denies the alleged patent infringement, and has filed for cancellation of the patent, with documents showing that the technology has been known since the 1940s," says the statement the company issued today.

Nike, in a statement responding to the court ruling, said, "This is just one step in the process. We will continue to aggressively protect our intellectual rights, including through the conclusion of this interim injunction proceeding as well as in a formal infringement case."

US to aid Beirut bomb probe amid Lebanon violence

Hussam Shebaro / Reuters

A Sunni Muslim gunman with a weapon rides a motorcycle through the streets of Kaskas in Beirut, Monday, after a night of tension.

By NBC News staff and wire reports

U.S. Secretary of State Hillary Clinton has pledged to help Lebanon investigate Friday's deadly car bombing in Beirut, as parts of the city were engulfed in violence that some observers say heralds the spread of civil war from neighboring Syria.?

She spoke with Lebanese Prime Minister Najib Mikati?on Sunday to reiterate U.S. condemnation of the attack - which?killed intelligence chief Brig. Gen. Wissam al-Hassan - calling it "heinous",?State Department spokesperson Victoria Nuland said.

"The secretary emphasized the United States' firm commitment to Lebanon's stability, independence, sovereignty and security," Nuland said in a statement.

PhotoBlog: Tension on Beirut streets as political crisis deepens

"She noted the importance of political leaders working together at this sensitive time to ensure that calm prevails and that those responsible for the attack are brought to justice,? the statement said.

Protesters rushed the prime minister's office Sunday in Lebanon, ripping up barbed wire and hurling rocks. The situation, which started as a peaceful protest, has become a standoff between protesters and the military. It has also triggered concern that Syria's civil war is spreading. NBC's Stephanie Gosk reports.

Nuland said Clinton and Mikati agreed that the United States "would provide assistance in the investigation of the bombing."

Syria blamed by Lebanese opposition
Opposition leaders and their supporters accuse Syria of being behind Friday's attack, and say Mikati is too close to Syrian President Bashar al-Assad and his Lebanese ally Hezbollah, which is part of Mikati's government.

Thousands turned out Sunday in downtown Beirut's Martyrs' Square for Hassan's funeral, which also served as a political rally. Violence erupted after an opposition leader demanded that Mikati step down to pave the way for talks on the crisis.

"The Syrian regime started a war against us and we will fight this battle until the end," said Anthony Labaki, a 24-year-old physiotherapist.

Huge blast explodes in a central Beirut street injures dozens, kills at least eight.

Sunday's clashes fed into a growing political crisis in Lebanon linked to the conflict in neighboring Syria.?Israeli news site Haaretz reported that the clashes "raised the specter of the nation once again becoming torn apart by civil war."

A group marched to the prime minister's office, then overturned barriers, pulled apart barbed wire coils and threw steel rods, stones and bottle at soldiers and police.

Security forces responded by shooting into air and firing teargas, forcing the protesters to scatter.

"Lebanon is in the eye of the storm," said Fawaz A. Gerges, head of the Middle East Center at the London School of Economics. "The fact that the protesters came close to storming the parliament shows how deep the crisis of the state is and how weak the leadership has become."

Fire exchanged in southern Beirut
On Sunday night, gunmen armed with rifles and rocket-propelled grenades exchanged fire in southern districts of Beirut, security sources told Reuters, and residents could hear the sound of ambulance sirens.

There were no immediate reports of casualties from the clashes in the capital, but in the northern city of Tripoli a 9-year-old girl was killed by a sniper and several people were wounded in clashes.

PhotoBlog: Violence erupts in Beirut after slain official's funeral

Gunmen have been patrolling the streets in Tripoli, scene of previous clashes between Sunnis and Alawites sympathetic to different sides in the Syria war.

Opposition leader Saad al-Hariri urged supporters to refrain from any more violence.

"We want peace, the government should fall but we want that in a peaceful way. I call on all those who are in the streets to pull back," Hariri said on the Future Television channel.

Sectarian tensions
Sunday's events highlighted how the 19-month-old uprising against Assad has sharpened deep-seated sectarian tensions in Lebanon, which is still scarred from its 1975-90 civil war.

Sunni-led rebels are fighting to overthrow Assad, who is from the Alawite minority, which has its roots in Shiite Islam. Lebanon's religious communities are divided between those that support Assad and those that back the rebels.

Hassan, 47, was a senior intelligence official who had helped uncover a bomb plot that led to the arrest and indictment in August of a pro-Assad former Lebanese minister.

A Sunni Muslim, he also led an investigation that implicated Syria and the Shiite Hezbollah in the 2005 assassination of Rafik al-Hariri, a former prime minister of Lebanon.

Damascus and Hezbollah have condemned Hassan's killing.

NBC's Paul Nassar describes the scene after a bomb killed 8 people in Lebanon Friday.

But mourners at Martyrs' Square on Sunday accused Syria of involvement and called for Mikati to quit. One banner read "Go, go Najib" echoing the slogans of the Arab Spring.

"We came for Lebanon's future," said mourner Rama Fakhouri, an interior designer. "And to show that we will not be scared."

Reuters and The Associated Press contributed to this report.

(AP) ? The San Francisco Giants and St. Louis Cardinals sure have something about going down to final games this postseason.

So it's fitting that the two previous World Series champions are playing a winner-take-all Game 7 for the chance to get back to baseball's biggest stage.

Ryan Vogelsong struck out a career-best nine in another postseason gem in his biggest start yet, and San Francisco staved off elimination for the second straight game, handing St. Louis a 6-1 loss Sunday night to force the deciding game in the NL championship series on Monday.

San Francisco's Matt Cain and St. Louis' Kyle Lohse are set to pitch in a rematch of Game 3, won by the Cardinals. There's a forecast of rain in the Bay Area during the day.

"You've seen us the last couple years, it seems, unfortunately, we don't win until we absolutely need to," Lohse said. "So, it's one of those things where we would love to have taken care of business the last two games. We haven't. ... It's time to get it done."

Lohse and manager Mike Matheny will need to get more from the Cardinals' offense if they're going to get a shot at defending their title against the Detroit Tigers, who have been waiting since completing a sweep of the Yankees on Thursday.

Marco Scutaro delivered a two-run double and Buster Posey drove in his first run of the series with a groundout in the first inning as San Francisco struck early to support Vogelsong. He followed up Barry Zito's stellar Game 5 outing in St. Louis with one of his own.

"I just kind of saw the way our team reacted the other night when Barry came out ... early and was throwing up zeroes," Vogelsong said. "I saw how our team was feeding off that. I just knew that I had to go out there and keep them off the board."

These wild-card Cardinals sure seem to like the all-or-nothing route in October, while San Francisco thrives on playing from behind.

In five games with their year on the line, the gutsy Giants have pulled out five wins in the postseason. After winning the one-game wild-card playoff at Atlanta, the Cardinals were on the brink against Washington before a remarkable four-run rally in the ninth for a 9-7 Game 5 win in the division series.

"Clutch performances are just guys playing normally in big spots. You can't be petrified by the situation, and neither of these teams have been," St. Louis' Lance Berkman said. "One of the things that makes Game 7's so interesting is that I don't think either of these teams is going to choke it away."

Pitching to chants of "Vogey! Vogey!" from the sellout crowd of 43,070 at AT&T Park, the right-hander didn't allow a hit until Daniel Descalso's broken-bat single to center with two outs in the fifth. Vogelsong struck out the side in the first and had already fanned five through two innings.

"This place is going to be loud, I can tell you that," Vogelsong said of Monday night.

Scutaro had no chance for a collision with Matt Holliday this time. In their first game back at AT&T Park since Holliday took out the second baseman with a hard slide in Game 2, Holliday was scratched about an hour before first pitch because of tightness in his lower back, and Allen Craig replaced him in left field.

It hardly mattered the way Vogelsong pitched.

The Cardinals managed their only run on Craig's two-out single in the sixth. St. Louis had gone 15 innings without scoring after left-hander Zito won 5-0 on Friday in Game 5.

Vogelsong had his second impressive seven-inning performance against the Cardinals in a week, allowing four hits and one run. He walked one in a 102-pitch performance and lowered his postseason ERA ? all this year ? to 1.42.

The 35-year-old Vogelsong toiled through the minors, Japan and even winter ball to finally pitch under the October spotlight for a chance at the World Series. His latest impressive outing put the Giants one win away.

"I just believe that it's my time," Vogelsong said.

After taking a 3-1 lead back home at Busch Stadium, Matheny's Cardinals will have to find some offense in a hurry if they want to get back to the World Series.

"Everything is on the line, and we couldn't be happier to have Kyle Lohse on the mound knowing that we're going to have another tough day with Matt Cain," Matheny said. "We've had some games where we stack on runs and then we go absolutely hitless, almost, for a while. But any day we know our offense can pull out quite a bit of production."

These Cards might just prefer close calls. Just like last year.

They won the NL's second wild card on the second-to-last day of the regular season, then won at Atlanta to reach the division series. The Cardinals rallied from a 6-0 deficit with a four-run ninth inning to stun the Washington Nationals 9-7 in Game 5 of the division series.

The Giants got to St. Louis ace Chris Carpenter again. The Cardinals' winningest postseason pitcher with 10 victories looked out of sync for the second straight start ? and he left with a nearly identical line as in his 7-1 Game 2 loss here last Monday, down to the hits, earned runs, unearned runs and innings.

Carpenter was done in by one big inning this time, too. He allowed six hits and five runs, two earned, in four innings.

Vogelsong reached on shortstop Pete Kozma's fielding error in the second, scoring Brandon Belt after he led off the inning with a triple. Scutaro came up two batters later and doubled home two more runs.

The 10 unearned runs allowed by the Cardinals are the most in an NLCS, according to STATS LLC ? topping the nine given up by the Braves in 2001 and Dodgers in 1985.

San Francisco never faced an elimination game in 2010 on the way to winning the World Series, but has had to go the distance in each of its first two postseason series this year. They became the first team in major league history to come back from a 2-0 deficit to win a best-of-five series by winning three straight on the road, doing it at Cincinnati.

"We're enjoying this moment. We know how to handle this situation," San Francisco third baseman Pablo Sandoval said.

They have Vogelsong along for this year's run.

"He was on top of his game again," Giants manager Bruce Bochy said. "He's probably been as consistent as any starter this year."

The Giants put pressure on Carpenter right away.

Scutaro drew a one-out walk and Sandoval doubled off the wall in center on a ball that eluded Jon Jay. Posey followed with a groundout to third to score Scutaro for a 1-0 lead.

Scutaro is batting .458 (11 for 24) during the NLCS.

"I don't really know, man," Scutaro said when asked to explain it. "Just excited to come to the field every day. ... Being in this opportunity, just being in the playoffs, is amazing."

While the Giants have won five straight games facing elimination this postseason, the Cardinals have won their last six dating to last year. This is the first time the NLCS has gone seven games since 2006, when St. Louis beat the New York Mets 3-1 at Shea Stadium. Carlos Beltran, now with the Cardinals, struck out looking when Adam Wainwright froze him on a curveball.

"This night, this moment belongs to every player in here and every fan in the stands," San Francisco right fielder Hunter Pence said. "There's still another story to be told, and we'll just have to wait and see if we're going to enjoy it."

NOTES: Giants 1B coach Roberto Kelly worked his first home game of the series after sustaining a concussion during the initial workout day Oct. 13 before the NLCS began. ... San Francisco is 5-1 when scoring first this postseason. ... The most unearned runs allowed in any LCS is 13, by the Angels in 1986 against the Red Sox.

Effect of the Hispanic vote in Vegas may not stay in Vegas

LAS VEGAS?This city has seen more political commercials than anywhere else in 2012, more than anywhere else in the history of presidential campaigns: 73,000 TV ads, according to The New York Times. In an election with only a handful of states that are considered up for grabs, the importance of Nevada's six electoral votes was highlighted when President Barack Obama chose Lake Las Vegas, a golf resort east of here, as the place to spend three days preparing for the first presidential debate.

The state's Hispanic voters are especially sought-after: According to Jim Messina, the Obama campaign manager, Latino voters are particularly crucial this year in Nevada, as well as in Colorado and Florida.

"As evidenced by the explosive growth among Latinos, Nevada offers a case study in the demographic changes that are transforming large swaths of the country and, in the process, reshaping the electoral map," David Damore, a political scientist at the University of Nevada, Las Vegas, told Yahoo News. "As older white voters die out, they'll be replaced by nonwhite voters."

There are more than?50.5 million Hispanics in the United States. That's a 43 percent increase in the Hispanic population in the past 12 years.

Yahoo News headed to Nevada earlier this year to find out what's on the minds of some of the most important voters in one of the most important states of Campaign 2012.

'I don't want to vote for the wrong guy. But it's really hard to find out who the wrong guy is.'

Austreberto Hernandez, known as "Asti," will be a junior at the University of Nevada, Las Vegas this fall, studying political science and business management. This summer he worked two jobs, registering voters for Mi Familia Vota and working as an intern for the?Latin Chamber of Commerce Community Foundation.

Hernandez and his six siblings were all born in the United States: Their father came across the border from Mexico looking for work?in the 1980s and became a citizen because of the?Immigration Reform and Control Act signed by Ronald Reagan in 1986.

"Ever since I can remember we would go from city to city looking for work," Hernandez, a registered independent, told Yahoo News. "Vegas was booming in the early '90s. It was just exploding. It was blowing out of the water. So everyone just headed here."

'I'm a Hispanic Republican. We're almost a minority inside a minority.'

Omero Alexandro Garza, who goes by "Alex," started working as a child in his family's truck stop. He taught middle school and then worked in real estate, starting out part-time at Century 21, then eventually founding his own real estate firm before moving on to other jobs in the industry.

"Without question, the primary issue for Hispanics is the economy," says Garza. "Immigration is near and dear to every Hispanic's heart. It's always going to be important to us. But more important is whether or not we have a job."

Garza's father came to the United States from Mexico as an undocumented immigrant. "He gained his amnesty under Ronald Reagan so it kind of just stems from there," Garza told Yahoo News, referring to his status as a registered Republican.

His house is underwater. He says he put down $80,000 on a $400,000 mortgage for a home that is now valued at $190,000. His wife has taken a job teaching English in Abu Dhabi, so the family is in the midst of moving to another country. "I mean I don't want to cry, but it's difficult because this was our hopes and dreams; this is where we raised our kids," Garza says. "It's the only house my children have known. It's difficult to go from stability within your family to instability and uncertainty."

He plans to vote by absentee ballot for Mitt Romney.

'If you look at the demographics, we are going to be the majority.'

Helena Garcia, 53,?styles herself "La Protectora." She's a real estate broker who also co-founded Latinos in Action, a pro bono advocacy group that tries to help people who think they have been cheated.

"If I only had to do real estate I would be very unhappy," Garcia told Yahoo News, adding that she prefers "going out into the community and seeing people's problems and devastation."

Garcia's family came to Las Vegas in 1956 from Juarez, Mexico. Her father worked as a dishwasher at a Mexican restaurant. Although she's a registered independent, she sides with the Democrats.

"In my opinion, most of the people in the Republican Party are racist and the majority of them want that more conservative view of 'Go back to where you came from,'" Garcia says. "There is no excuse not to pass the DREAM Act when these kids are going to better our country; they are going to bring more education to our country. Most of them are fully bilingual so you bring more culture and languages to our country. There is no other reason not to pass the DREAM Act except for racism."

Bob Sacha is a multimedia producer, a documentary filmmaker, a photojournalist, an editor and a teacher. Zach Wise is an interactive producer, a filmmaker and a professor at Northwestern University. Earlier this month, they talked to small-business owners along Colorado's Colfax Avenue. In July, Bob Sacha and Miki Meek traveled to Northern Virginia to?talk to Mormons about what a President Romney would mean to them. In March, they drove Ohio's I-71 and talked to Republicans before Super Tuesday.

How to Make It in America ? 7 Tips To Help Your Startup Enter The U.S. Market

SteliEditor?s note: Steli Efti is the Co-Founder/Chief Hustler of ElasticSales and an advisor to several startups and entrepreneurs. You can follow Steli on Twitter here. Selling to the U.S. market can be a significant milestone in the life of an international technology startup. The U.S. is the largest market in the world, and a successful foothold here can mean dramatic growth as well as further credibility of your company. But selling to U.S. businesses can be difficult especially if you?re based overseas. Having successfully introduced international technology companies to the U.S. market, we?ve learned several lessons, listed below, to help companies abroad sell in the U.S. Find markets that are not yet saturated by your competitors. U.S. competitors will have a geographic advantage, so pick an industry vertical, or regional market, where you can introduce your product and gain rapid traction. Identify Early Market Signals. Track early signals that the U.S. market is even interested in your product or service. Do you have signups on your website or requests for information from U.S. customers already? Set up a U.S. page where customers can sign up to be on a waiting list and run a basic online marketing campaign to promote it. If you see interest in your product or service from the U.S. market, you know there?s an opportunity to push further into the market. Experiment with pricing, but don?t get caught up in a price war. ?International companies often differentiate themselves from U.S. competitors by offering similar technology at a lower price. However, cheaper doesn?t always compute as better; many customers associate a lower price point with lower quality. So rather than racing your competitor to the lowest possible price point, pitch your product as providing a superior solution at a price that can actually support the growth of your company. Get introductions from your existing clients. Many of your customers may already operate in the U.S. and have industry contacts that could also benefit from your product or service. Ask them for introductions to new customers. Tell them you?re entering a new market, and as a long term or successful client, you?d appreciate a connection with any industry contacts that would also benefit from your solution. Asking your existing customers is an easy way to find early adopters in new markets, but a tactic that many businesses simply fail to do. Leverage the brand of a notable customer.

'Rust and Bone' named best picture at London fest

LONDON (AP) ? "Rust and Bone," Jacques Audiard's soaring story of love, loss and killer whales, was named best picture at the London Film Festival on Saturday.

The movie is a thriller-cum-melodrama about the unlikely relationship between a bare-knuckle boxer (Matthias Schoenaerts) and a whale trainer, played by Academy Award winner Marion Cotillard, who suffers a tragic workplace accident.

The president of the award jury, British playwright David Hare, praised it as "a film full of heart, violence and love."

French filmmaker Audiard won the same award at the London festival in 2009 for his prison drama "A Prophet."

American director Benh Zeitlin took the best debut feature prize with his atmospheric bayou saga "Beasts of the Southern Wild." Juror Hannah McGill praised the "daringly vast, richly detailed" film, which has won wide praise since its Sundance Film Festival debut earlier this year.

The trophy for best British newcomer went to Sally El Hosaini for "My Brother the Devil," the story of British-Egyptian brothers struggling with conflicting loyalties and identities in modern-day London. The best documentary prize went to Alex Gibney's "Mea Maxima Culpa: Silence in the House of God," an investigation of sexual abuse within the Roman Catholic church.

Director Tim Burton and actress Helena Bonham Carter ? real-life partners as well as creative collaborators ? received career honors known as British Film Institute Fellowships during an awards ceremony at London's Banqueting House.

Founded in 1957 to show the best of the world's cinema to a British audience, the London festival has in recent years tried to carve out a place on the international movie calendar with bigger pictures, more glittering stars and more high-profile awards.

Highlights 12-day festival included Ben Affleck's Iran hostage drama "Argo," Michael Haneke's Cannes Palme d'Or winner "Amour," Rolling Stones documentary "Crossfire Hurricane" and Roger Michell's "Hyde Park on Hudson," a comedy-drama with Bill Murray as U.S. President Franklin D. Roosevelt.

The festival wraps up Sunday with a gala screening of Mike Newell's adaptation of Charles Dickens' "Great Expectations," starring Bonham Carter and Ralph Fiennes.

Syrian opposition skeptical of 'feeble' ceasefire plan

A look back at the violence that has overtaken the country

United Nations peace envoy Lakhdar Brahimi hopes to persuade Syria?s President Bashar al-Assad to comply with a four-day truce during the Muslim holiday marking the Hajj, which begins Oct. 25, but many in the opposition remain skeptical about his plan.??

Brahimi, who arrived in the capital Damascus on Friday afternoon, will meet Foreign Minister Walid al-Moualem on Saturday morning, said the U.N. spokesman in Damascus, Khaled al-Masri. He did not say whether the envoy would meet Assad.

"We will talk about the ceasefire and the Syrian issue in general. It is important to decrease the violence - we will talk with the government and political parties and civil society about the Syrian issue," Brahimi told reporters upon arrival.

The violence showed no sign of abating, with opposition activists reporting heavy street clashes in Aleppo, Syria's biggest city, and intensified army bombing of towns along the strategic north-south highway. U.N. Secretary-General Ban Ki-moon and Arab League chief Nabil Elaraby backed a ceasefire. "The longer the violence lasts, the more difficult it will be to find a political solution and rebuild Syria," they said in a joint statement.

Turkish Foreign Minister Ahmet Davutoglu called for all sides to observe the three- or four-day ceasefire.

"It is important that the Syrian regime, which bombards its own people with fighter planes and helicopters, halts these attacks immediately and unconditionally," Davutoglu said in Ankara.

Iran also backed the ceasefire call but added that the main problem in Syria was foreign interference - a reference to support for the rebels by Gulf Arab states, the United Sates and other Western powers, and Turkey.

"We consider the establishment of an immediate ceasefire an important step in helping the Syrian people," Iranian Deputy Foreign Minister Hossein Amir Abdullahian said, as quoted by Mehr news agency.

"Syria has taken important steps against terrorism and foreign interference and is pursuing political reforms and the security of the country."

Most opposition forces doubt Syria will comply with what they see as another toothless initiative. The largest opposition group, the Syrian National Council, has thrown its support behind the initiative but will hold the Syrian government responsible for any violation.?

?The last time [a cease fire was agreed upon], the Syrians [who support Assad] violated it the first day. I hope this time they will respect it,? says Khaled Khoja, council representative and spokesman in Turkey. ?I hope this time they will accept it, allow people to express their ideas peacefully, release prisoners and withdraw their forces from cities.?

Although Khoja says the Free Syrian Army, an umbrella organization for defected soldiers and armed opposition, has conditionally accepted the truce, its fighters will respond if attacked first by Syrian government forces. Khoja, like many in the opposition, worries that the initiative will fail because there is no penalty to compel the regime to observe the ceasefire.???

Syrian helicopter reportedly downed by rebels over Idlib

?There must be a mechanism in order to make the Syrian regime comply. If there is no mechanism, the Syrian regime will continue to do the same.?

Rafif Jouejati, the Washington, D.C. based spokeswoman for the Local Coordination Committees, the largest activist network inside Syria, believes the truce is destined to fail.

?It is a feeble attempt by the international community that has no timeline, no consequences for failure to observe it, no consequences for continued bombardment. Since there are no consequences, any plan will fail and [Assad] will continue to kill civilians. It carries the same weight as a U.N. condemnation," she said.

U.N. Special Envoy Lakhdar Brahimi is calling for a four-day cease-fire in Syria next week to mark a Muslim holy day. NBC's Stephanie Gosk reports.

"Unless there are actual consequences, it is meaningless. I have relayed messages to Brahimi from the protesters: ?Welcome to Syria on your mission impossible. Why didn?t you send Tom Cruise?? It is an insult to the people who go out everyday and brave the bombing and bullets.?

Syrian government forces strike rebel held towns

Jouejati said this was the fourth attempt at brokering a ceasefire deal. ?Assad has proven he is not going to look for political solutions. He has confirmed it through word and deed and will continue his security solution.??

A Britain-based monitoring group, the Syrian Observatory for Human Rights, doubts Brahimi?s ability to influence armed elements, whether they're on the opposition or government side. Brahimi cannot control Jebhat al Nusri, a jihadist militia, said director Rami Abdulrahman. ?"He cannot control what is happening on the ground," Abdulrahman said. "He doesn?t know how many Syrian troops are on the ground.?

More than 30,000 people have been killed over 19 months in the intensifying conflict.

More weapons in Syria could trigger 'all-out war'

There is little agreement among the many armed rebel militias who are temporarily unified by the desire to topple Assad. Although the Free Syrian Army reportedly supports the truce, a senior commander in Daraa, a Syrian city on the border with Jordan and the starting point of the revolution, told the British Guardian newspaper, ?Brahimi?s proposal for a truce is stillborn.?

?His project does not have any hope. Even if [exiled FSA leader] Riad al Assad accepts the truce, we will not. We will continue to fight," Staff Col. Ahmad Fahd al-Nimah, commander of the military council in Daraa, told the Guardian. "We represent those fighting on the ground in Syria. No one outside Syria can tell us what to do.??

An activist reached by phone in the same city told NBC News the people in the streets of Syria do not accept the ceasefire. Da?il, who gives only his first name for security purposes, believes the regime will use the ceasefire to fortify military positions.?

?Before, when the Arab League and U.N. sent in peacekeeping observers, peaceful protesters were still being shot at. How can there be a ceasefire now, when there is nobody on the ground to observe it? No way will it work. Right now there is a roadblock with a sniper in front of me. If I walk out I will get shot. There is no way to stop the fighting. It is going on all the time,? he said.?

Da?il went on to express the public?s cynicism toward the initiative: ?There is a local saying here: If somebody tries something and fails, he is crazy if he tries the same thing again.??
